Adoray Home Health and Hospice is currently recruiting for a Full time LPN to join our Home Health Team.

LPN Duties:

•Makes independent nursing visits on assigned patients.

•Implements care plan and institutes appropriate revisions as directed by the RN on an ongoing basis.

•Ensures physicians orders are carried out in a timely manner as assigned.

•Reinforces teaching to patients and/or families as provided by the RN.

•Assists patients in learning appropriate self-care techniques.

•Prepares equipment and materials for treatment, observing aseptic technique as required.

•Consults with RN regarding observations made which may necessitate referrals to other appropriate health care workers and/or agencies. •Complies with universal precautions and standard infection control protocols and general safety policies and procedures.

•Maintains accurate, complete documentation of nursing activities on medical record.

•Ensures documentation is on the medical record within 48 hours following the provision of care.

•Updates schedule on a daily basis.

•Provides appropriate office staff with notification of visits made on a daily basis and turns in written verification of visits on a weekly basis.

•Maintains close communications with RN case manager regarding general observations of patient and patient/family status.

•Communicates with RN case manager regarding observations in which additional service may be of benefit as appropriate.

•Promptly informs the RN, physician or other persons participating in the patients care of changes in the condition and needs.

•Participates in case conferences at lease once per month for Home Health and/or IDT meetings for Hospice as needed and contributes to care plan revisions of all patients reviewed Based on knowledge and experience
